Analyzing Consumer Assessment of Healthcare Providers and System Survey Data

Care Management Plus (CM+) is a primary care model that uses information technology and care managers to reduce hospitalizations and to improve the quality of healthcare provided for seniors and patients with chronic illnesses. It works on enhancing primary care by providing a team based approach that is centered on a care manager who addresses the needs of high-risk patients. Physicians, health program officers and data analysts work on improving the CM+ intervention program to achieve better outcomes for these patients. The presenter will provide insight on the general process involved in the identification of the problem and the development of the intervention programs. Further, the presentation will highlight the importance of quantitative and qualitative analysis of the Consumer Assessment of Healthcare Providers and System (CAHPS) survey data to achieve the research goals in the intervention programs.
