Incoming forms for Graduate Students

Requirements for students in the MAT, Teacher Leadership and MAT, Mathematics programs

The My Health Connection portal must be used to complete the required health forms. Your user name and password for the portal is the same to access my.mtholyoke. Covid vaccinations and boosters are required.

Immunization Records:

• COVID-19 vaccination** required: Entering the dates online of your primary series and booster, and submit proof of vaccination through our portal or email medical-records@mtholyoke.edu. Please view the instructions regarding vaccination.

Vaccination exemption forms (if applicable)

• Medical exemption
• Religious exemption
