Break Address Update Form

If you will be remaining in the U.S., please list your physical break address below and the date when you will be moving there. If you are going home or will be outside the US for the break, you do not need to complete this form. 

Note: Failure to inform the McCulloch Center for Global Initiatives of your break address if moving off campus will mean that you are out of legal immigration status. We will not be mailing anything to you at this address nor will the College forward your mail unless you change your address with the Registrar's Office.
